 Working as an advocate for Connect for Health I have found that this organization practices high standards of professionalism especially when it comes to protecting the privacy and well-being of their clients. (C4H) distinctive characteristics of public health professional practice are community-focused as the program has strategies implemented that aim to reduce social determinants of health for residents in the Rhode Island community. Moreover, the focus of the program is to provide resources, support, and care for those in need. Additionally, (C4H) is population-based as this program is dedicated to helping individuals in Providence Rhode Island, and provides resources to individuals of all ages through their social referral program based out of Hasbro Children’s Hospital and the Center for Primary Care (Zhu &Ahluwalia, 2020).

    Implementing the basis of public health and professional practices through population-based characteristics is to improve health outcomes of specific groups and identify risk factors for certain patient populations across all groups which is another key characteristic (C4H) embodies in keeping their clients from facing difficult arising situations. (C4H) looks at all levels of prevention from a system, community, and family standpoint that motivates and promotes health, detects, and treats problems with the use of collaborative efforts to help resolve challenges. When there is a level of awareness of what is transpiring within a community along with organization, policy, law, system changes, and individual belief, knowledge, and behaviors, professional practices must be implemented to add to the overall opportunities, wealth, and privileges of improving population health status (Thrift & Sugarman, 2019).
