Student #1: The IOM report “The Impact of Climate Change in Uganda” involves an adolescents explaining in grave detail the effects of climate change that have turned seasons around with the country experiencing shorter and longer rains and harsher droughts compared to their normal stable rainfall patterns. Thus, having a great impact on food and water supply, making it hard to families to survive in poor conditions. Its unfortunate to hear this child lost his family during these tragic climate conditions. Under these circumstances, the IOM implemented a project designed to understand environmental migration along with climate change and will provide technical support to enable villages to develop action plans for migration and displacement (“The Impacts”, 2021).

As healthcare providers we witness first-hand the harmful effects of climate change, from exacerbated lung and cardiovascular conditions due to poor air quality, increases in illnesses from extreme heat, storms floods and droughts causing displacements and threats to food and water sources. Along with IOM, nurses have opportunities to advocate for changes that will help protect the environment and increase awareness while promoting best practices. (Schnur, 2019).

Student #2:The article I chose is “IOM Provides Emergency Help to Thousands of Haitians Affected by Earthquake.” The article talks about how there was an earthquake on August 14th. Among 800,000 people affected by the earthquake, thousands are internally displaced, and hundreds are still missing (IOM Provides Emergency Help to Thousands of Haitians affected by Earthquake, 2021). More than 12,000 have been injured, straining an already fragile health system. According to official figures, more than 52,000 houses have been destroyed and 77,000 severely damaged, including essential schools and hospitals, and 2,200 people have died (IOM Provides Emergency Help to Thousands of Haitians affected by Earthquake, 2021).

As nurses, we can help by assist with food, shelter, attending to anyone who has been injured, getting hygiene kits with the basics of soap and toothpaste. Provide support and help the public regain confidence in rebuilding their lives and living through hard times. In Haiti, you also have to take into special consideration for women and girls who are more vulnerable to sexual abuse and exploitation in an emergency. Just being there and helping in time of need and showing that you care can impact their lives and get them through this difficult time (IOM Provides Emergency Help to Thousands of Haitians affected by Earthquake, 2021).

Student#1: Patient engagement is an important part of their plan of care throughout their hospital stay. Patient participation causes improved health outcomes, enhanced quality of life, and delivery of more appropriate and cost-effective services, if patients are regarded as equal partners in healthcare, they would actively participate in their own health care process, and more carefully monitor their own care (Vahdat, 2014).

A time when I actively encouraged my patient to be involved in their own care, was recently. I had a patient who was extubated within a few hours from the start of shift and I’m pretty sure all of us have encouraged the use of an incentive spirometer. Unfortunately, my patient developed hospital acquired pneumonia due to excessive intubation and ventilation. Incentive spirometer is an important part of aftercare, once a patient has been extubated, to optimize ventilation and prevent infections. I educated my patient and encouraged use every time I did my neuro assessment which is every hour in the ICU. This improved their lung expansion, mucus clearance and overall ventilation. Patient centered care is supported by creating a framework that provides a better understanding of patient engagement and applying that to patients responsiveness by involving them in their plan of care.

Student #2: I encouraged a patient to be involved in her care when I created an account in the facility’s e-health platform and synchronized all her health information. The patient did not understand how the portal worked and whether she could communicate to her practitioners at any time. She traveled a long distance to talk to her doctors and communicate her needs. Hence, I realized that a portal could help promote care centered on her by enhancing accessibility and reducing barriers to communication. For instance, the patient often neglected her needs because the process of finding care was tedious. The portal allowed her to manage her condition and talk to her physician at any time. She would easily look at her records, ask questions, and raise concerns through a platform available to her and the primary physician. Even though our facility did not actively use the portal, her primary physician agreed to make it a priority to increase her voice in the treatment and management process.

The portal made a significant difference to the patient’s health since she communicated constantly and asked questions whenever she did not understand the illness management. She would post an update, and the primary physician would respond promptly or provide recommendations to help her. With time, she trusted her primary physician more and became confident in her ability to make decisions about her health since she could easily access strategies and recommendations. The patient’s hospital visits reduced considerably, and she was not admitted for the first time in three months. She seemed more knowledgeable about her condition and recovered well.
