Research and locate an example of an Advance Directive form. Find a blank form, not just an article about Advanced Directives, unless there’s a blank form attached to it. Summarize the Advance Directive, and identify whether it is Substantive or Proxy. In the Works Cited, be sure to include your Advanced Directive, properly formatted including its title, date of publication, and link.
